1. Birth certificate or other verification of age – child must
be 5 years of age on or before September 15, 2007.
2. Verification of up-to-date immunizations – 5 DPT, 4 Polio,
3 Hepatitis B,
2 MMR, 2 Varicella (at least 28 days apart) OR history of disease documented
on
immunization record from doctor’s office or lab confirmation.
3. proof of residency – public service/phone bill, lease agreement,
real estate contract
All other new students in grades 1-5 must have verification of grade
placement, birth certificate, proof of residency, proof of immunization,
and complete mailing address of previous school.

Note: To keep class sizes balanced, Cotton Creek walk-in students will
be placed in both morning and afternoon classes.
LUNCHES
School lunches are provided each full school day with choices of entrees,
side dishes, fruit and vegetable bar, and milk. Students bringing sack
lunches from home may purchase milk. Our lunch payment system is computerized,
therefore, students are not given a ticket. Lunches may be purchased
in daily, weekly or monthly increments. Lunches are $1.90 daily for
elementary students (price is subject to change).
